The primary purpose of this position, under the supervision and/or direction of the Health Services Director, is to assist the Health Services Director, Director of Nursing, the Resident Services Director, the Care Management Team, and the Executive Director in the coordination of services described in the independent resident’s service contract. As the Home Care Coordinator, the primary purpose of your position is to supervise the implementation of the plan of care for Independent and Assisted Living residents enrolled in Givens Highland Farms Home Care in accordance with current federal, state and local regulations that govern home care. You are to assure that the highest degree of quality care can be provided to our residents at all times.
Candidate must be currently licensed in the State of North Carolina as an RN, have current CPR certification or obtain within 3 months, and preferably have previous experience in home health/geriatrics/long-term care. Candidate must also have previous experience managing a home care or home health department.
